Your First Visit
Prior to your visit, you will receive an appointment confirmation text or email from us. If this is your first visit, you will also be prompted to complete your new patient intake forms. Clicking the link to confirm your appointment will automatically lead you to complete your patient registration and prompt you through the entire process. It’s easy to fill out, and we will remember your answers for your next visit, so you only have to make necessary changes in the future.
Registering before your visit helps us serve you better and faster when you come into the office. We understand that sometimes it can be difficult to fill out forms on a mobile device or computer, which is why we do offer the option to do it on our iPads in the office. Please arrive at least 15 minutes early to fill out your pre-visit information on our electronic iPads if you cannot complete it before your appointment
